A new Arcus healthcare survey of 1328 patients earlier this month has found hat the single greatest concern raised by respondents is inadequate publicly funded home care services. The top issues identified all relate to inadequate publicly funded home care.


Hospital-to-home care transition

A key strategy will be to focus on improvements in the hospital-to-home transition navigated by about one million patients every year. The study focused on identifying what affected the patients’ experience during their discharge and transition to home.

Patients are currently discharged home from hospital feel that once they leave the hospital that they’re disconnected from the entire health care system.

Ontario has created local Ontario Health Teams that bring hospitals and home care agencies together under a single umbrella. These teams seek to identify where the gaps in services are and will be able to fill them. The creation of the Ontario Health Teams marks a shift in how home care is coordinated and provides an opportunity for addressing the issues raised by the survey. Hospitals can play a key role in improving patients experiences by creating follow up mechanisms and feedback loops to address post-discharge issues.
